Article Title: Characterizing a Left Ventricular Pseudoaneurysm With Photon-Counting Detector CT
Journal: JACC Case Reports
doi: 10.1016/j.jaccas.2025.106238
Figure Legend Snippet: PCD-CT Visualizing a Left Ventricular Pseudoaneurysm and Inferior Myocardial Infarction (A and B) A left ventricular pseudoaneurysm with intraluminal thrombus (blue arrows in B) is visualized, accompanied by a significant amount of pericardial effusion (asterisk in B). (C) Late enhancement acquisition using iodinated contrast shows an increased signal of the inferior myocardial wall apical of the pseudoaneurysm on a calculated iodine map (orange arrow) and (D) an extracellular volume map (arrow), indicating myocardial fibrosis. On the iodine map, an increased signal surrounding the aneurysm is visualized (white arrows in C), suggestive of pericardial involvement. CCTA = cardiac computed tomography angiography; ECV = extracellular volume; LV = left ventricle; PA = pseudoaneurysm.
Techniques Used: Computed Tomography
Figure Legend Snippet: Photon-Counting Detector CCTA Photon-counting detector CCTA image demonstrating a pseudoaneurysm of the inferoseptal myocardial wall, occlusion of the right coronary artery (blue arrow), and pericardial effusion (asterisk). CCTA = cardiac computed tomography angiography; PA = pseudoaneurysm.
Techniques Used: Computed Tomography
Figure Legend Snippet: Short-Axis Comparison Between CMR and PCD-CT Findings The basal short-axis slices (upper row) show late enhancement surrounding the pseudoaneurysm, while the mid slices (middle row) show late enhancement of the inferior wall below the pseudoaneurysm. The apical slices (lower row) do not show any enhancement of the myocardium. A stack artifact in the PCD-CT images seems to cause a horizontal line of increased ECV in the mid/apical short axis slice. T2-weigted CMR images do not include a hyperintense signal in the infarcted myocardium, indicating the absence of myocardial edema and confirming the chronic stage of the infarction. A hyperintense signal of the pericardium is visible, consistent with active pericarditis. CMR = cardiac magnetic resonance imaging; ECV = extracellular volume; LGE = late gadolinium enhancement; PCD-CT = photon-counting detector computed tomography.
